The form prepares a message in your own email app. It does not submit or store your enquiry details on this website. You choose whether to send the email.
Sharing sensitive information
Please leave medical records, NDIS numbers and detailed participant information out of the initial enquiry. Contact our team to agree on a suitable way to share information needed to discuss care.
Email and external services
If you send an email, your email provider handles delivery. This site uses a font hosted by Google, so your browser may connect to Google to load it. External links take you to services with their own privacy practices.
