Complex care support

Find the support
that fits your needs.

Tell us what help is needed. We’ll talk through the right services and next steps.

Match the task, not just the diagnosis.

We invite referrals for complex disability support, including personal care, community nursing, higher intensity support and changing care arrangements. ABI and FND are two distinct areas of focus within this broader service. Service suitability depends on the actual support requested, location, available workforce and the relevant care needs.

Area to discussWhat a referral review needs to clarify
ABI supportDaily routines, communication preferences, personal assistance and rehabilitation guidance.
FND supportDaily goals, current treating plan, assistance required and agreed response arrangements.
Community nursingThe nursing task, clinical instructions and appropriate professional involvement.
High intensity personal supportThe specific task, individual plan, required skills and registration scope.
Personal & household supportDaily activities, privacy preferences, assistance level and timing.
Community participationGoals, access needs, practical support and current opportunities.
Transitions & shared livingThe existing arrangements, proposed changes and each provider’s responsibilities.

High intensity support needs a specific review

Describe the particular care task rather than requesting “complex care” alone. Ask the team to confirm whether the support falls within its current service scope, registration requirements and the team’s skills.

For registered providers, high intensity daily activity supports must fall within the scope specified on the provider’s registration certificate. Relevant worker skills and participant-specific training must also be considered.

Diagnosis and rehabilitation

These referral pathways concern disability support. Do not assume a referral includes neurological diagnosis, a specialist rehabilitation programme or a particular therapy discipline; confirm those arrangements separately.

Funding

A diagnosis of ABI or FND does not automatically establish NDIS eligibility or funding for a particular support. Funding and service arrangements need to be considered individually.
