Complex care

Care with a fuller picture.

Complex care is about the whole person: their daily life, care needs and the people around them. We welcome enquiries across a range of complex disability and support needs.

Understand the full picture.

Complexity can come from physical support needs, nursing requirements, communication, several services working together or changes in care arrangements. ABI and FND are distinct areas of focus; our complex care enquiries extend beyond those diagnoses.

Your care may involve personal support, nursing and several people or services. We start by talking about how those needs fit together, what is already working and what needs to change.

The aim is to understand your requirements before agreeing on a practical, suitable service. Your priorities and the people you choose should be part of that discussion.
