Our clinical approach

Clear roles.
Care you can understand.

Know who is involved, what support is agreed and who to contact with a question.

Good care needs clear communication.

When several people are involved in care, it helps to know who does what. Before support starts, we’ll talk through the care being requested and how it fits with the person’s existing team.

The people involved

Ask us who would be your main contact, which clinicians would be involved and who is responsible for each part of the agreed support.

The right skills

Tell us about the tasks that need help, any care instructions and the person’s communication needs. We’ll discuss the skills needed and whether the service is a good fit.

Questions and changes

Agree on who to contact with a question, how changes will be shared and what to do if there is a concern. Your preferences and the people you want involved are part of that conversation.

Your voice matters

You can ask questions, raise a concern or request a review of your support. Talk with the team about how you would like to stay in touch.

Let’s talk about your support.

We’ll help you work out the next step.
