What to include in an initial enquiry
- Your name, role and contact details
- The service or support being requested
- The suburb where support is needed
- The preferred start date or planning timeframe
- Whether the person has agreed to the enquiry
Keep the first enquiry general. Please do not enter medical records, NDIS numbers, detailed clinical histories or other sensitive participant information into this form.
What happens next?
